When reporting an installation, ALL installers must submit a Form T to the Department within either 3 or 7 days. You can submit and pay for your Form T online, or you can fill out and print a hard copy to mail to the Department.

- (Suggested) 1118 – Procedures for Salvaged Manufactured Homes
- Form 1026 – Form T – Notice of Installation to the Manufactured Housing Division (or Form 1069, if the installer has a Provisional license)
- Form 1019 – Site Preparation Notice (Used Homes only)
- Form 1060 – Improper Site Notification to Consumer (used homes only, and only if the site shows evidence of one of these characteristics)
