MHD Announcement: Fillable Forms Available on the Web

In our continued efforts to improve efficiency, the following forms are now available as fillable forms in PDF format on the TDHCA Manufactured Housing Division’s (MHD) Web page. This feature enables the user to fill out the form, save it, and print it so it can be submitted to the MHD.

• Application for Statement of Ownership and Location;
• Release/Foreclosure of Lien;
• Statement of Inheritance;
• Installation Report (for Installers to report each installation);
• Central Tax Collector Form; and
• Consumer Complaint Form.

In the very near future, the MHD will make more fillable forms available online and include the feature to fill out forms, make payment, and submit online.
