Brokers must keep all forms that a Retailer would keep (depending on if the home was NEW or USED), noted in the appropriate list above, PLUS the following Broker disclosure:

  • Form 1055 – Broker Disclosure Form

  • Form 1002 – Name and Address Change Request to the Department
  • Form 1016 – Assignment of Certificate of Deposit as Security
  • Form 1025 – Continuous Manufactured Housing Licensing Surety Bond
  • Form 1031 – Criminal History Affidavit
  • Form 1048 – Estimate for Reassigned Warranty Work from the Consumer Claims Program
  • Form 1064 – Instructions to Third Party Closer from Retailer
  • Form 1081 – Adding or Deleting a Related Person to a License Record
  • Form 1082 – Industry-Requested Inspection of a Manufactured Home
  • Form 1084 – Industry Training Request and Billing Statement
  • Form 1104 – Warranty Order Extension Request
  • Form 1109 – Salesperson’s Sponsorship Change Request
  • Form T – Notice of Installation to the Manufactured Housing Division
  • Provisional Form T – Notice of Installation for a Provisionally Licensed Installer

Required In-Office Signage

  • Form 1059 – Notice of Licensed and Bonded Location (“Consumer Protection Information Sign”)
    Note: At each licensed location, the retailer is required to conspicuously display the consumer protection sign linked above.
  • RETAILER LICENSE and the license(s) of SALESPEOPLE YOU SPONSOR: At each licensed location, all retailers must display their current license for that location, and the current license of each salesperson who works from that location.
